The FINDconnect™ dashboard fulfills Help Me Grow® national fidelity metrics through real-time data reporting. It offers instant results to account when a family’s needs are met in the initial screening. Linkage metrics are available to note if a family connects to at least one community resource on their action plan. FINDconnect™ reporting shows the number of families successfully screened for Opportunity Assessments and Social Determinants of Health along with Ages and Stages Questionnaires.

The dashboard instantly reflects the number and type of incoming referrals to Help Me Grow® and the number of resources provided to families. Resources a family utilizes are quickly tallied along with categorization of whether action is taken or declined and why a family declines a resource. Customizations have been incorporated to not only fulfill Help Me Grow® fidelity and reporting metrics but also individualized quality management needs to support a higher standard of excellence in serving Help Me Grow® families.
