Realtime Loop Closing

Understanding if families are utilizing resources is extremely important to providing the best quality of care. FINDconnect™ includes a variety of methods for both navigators, caregivers, and community resources themselves to close the loop on resource referrals. Shown here is the SMS (text message) method of closing the loop that involves the caregiver texting a unique code from an action plan to a local area phone number. FINDconnect™ will automatically close the loop, and transfer any additional responses into FINDconnect™ for the navigator to read and discuss during follow ups.

Shifting The Paradigm

FINDconnect™ was proudly founded in the Bay Area at the UCSF Benioff Children’s Hospital Oakland. As the largest pediatric health care providers UCSF had an unprecedented opportunity to make social screening and resource connections in a health care setting a reality for all children. To do this, UCSF recognized the need to shift the paradigm of traditional medical care to more comprehensively manage determinants of health as a standard of care.
