The FINDconnect™ mobile app was designed with the community in mind. Caregivers can find resources in their area without needing to sign up. Depending on their proximity to FINDconnect™, caregivers can enroll themselves and either have care automatically transferred to a local navigator, or self assess and create actions plans all on their own! The options available are made to support your clinic or call center how you need it when adopting FINDconnect™.

Helpful Guidance
The FINDconnect™ mobile app makes sure the caregiver is guided through the entire process. Depending on how your clinic wants to work with caregivers in the community, the app will guide them to where they need to be, and how they need to engage with care. This includes fully navigator owned enrollment with the caregiver, caregiver self enrollment with handoff to navigator at clinic, and fully self enrollment and assessment by the caregiver themselves. FINDconnect™ provides helpful features to facilitate this kind of interaction within the app.



Easy Enrollment and Assessment
Should your organization desire caregivers in the community to self enroll and assess before being handed off to a navigator for continuance of care, the FINDconnect™ app easily handles that. Caregivers can sign up for an account, add their info, and info on any children they care for. They can then self assess and fill out any appropriate surveys related to their children’s age and needs right in the app. If complete self service is desired, they can also create action plans based on relevant resources in their area.



Loop Closing and Communication
Adding to the available ways to close the loop on utilized resources and communication, this can all be done within the app to decrease time for follow ups and communication with the caregiver.
